It is generally not considered safe in pregnancy, especially in the third trimester where it has been associated with neonatal complications including respiratory, gastrointestinal, and feeding problems, seizures, and withdrawal symptoms. Avoid sibutramine (Meridia) with this medicine and other SSRIs. The usual starting dose is 20mg once a day. Don't use with MAOIs or tryptophan (not even within 14 days of each other). May be potentiated by CYP3A4 inhibitors and antagonized by carbamazepine.
